Support Coordination - Level 2
This level helps you build confidence and skills to direct your life, including:
Plan Understanding: Helping you understand your funding and put your approved supports into action.
Provider Connection: Connecting you with suitable local providers, therapists, and community services.
Capacity Building: Teaching practical skills and building your confidence so you can manage your plan by yourself over time.
Network Maintenance: Resolving service gaps, handling adminstrative taks like service agreements, and keeping multiple providers working well together.
Review Preparation: Monitoring budgets, checking that your services meet your goals, and help prepare for plan re-assessments.
Specialist Support Coordination - Level 3
This is a higher level of support coordination if you have a more complex situation.
A specialist coordinator makes sure you have consistent services during any challenges in your support environment.
Some common situations that may apply:
Justice system involvement: Leaving prison, on parole, or management court orders alongside your NDIS plan.
Housing instability: Experiencing homelessness or at immediate risk of losing your accommodation.
Complex mental health: Facing frequent hospitalisations or severe psychosocial crisis episodes.
Multi-system engagement: Interacting simultaneously with the NDIS, child protection, health, housing, and justice systems.
